- Staff Pension Application Forms.
- Staff Family Pension form to be submitted on the death of an employee / IOB pensioner.
- Membership form under IOB Retired Employees Medical Assistance Scheme (REMAS)
- Nomination form for Mentally Retarded Child under Staff Family Pension
For reaching these forms, the following procedure has to be adopted:
Go to front Page of our IOB Online —> click Print Your forms (available under “others” column on the right side corner)—> click PAD forms —> click sl.nos.9,10,11 and 12, as the case may be.
